Wholesale Customer Application Form
If you would like to continue with setting up a wholesale account you will need to supply us with the following documentation.
- Resellers Certificate/ Physicians licence
- Tax Identification Numbers
Once you have supplied us with scanned copies of the required documentation we can then set up your account and you can begin ordering at the wholesale rate of $27.20 per box.
Please note that our product cannot be resold online under any circumstances, only in store.
Please be aware that it is OmegaBrite Wholesale policy that the minimum order for wholesale customers is 25 boxes or more in each order for our Gelcap products and 12 bottles or more per flavor for our Kids Products. This is policy is non negotiable and orders under stated quantities will be charged at the consumer rate of $39.99 per box and $44.99 per bottle.
